Health Education / Disease Management Programs
DIABETES EDUCATION
Osco Diabetes Control Network
Includes blood glucose monitoring, medication management, carbohydrate counting, dietary management, exercise, relaxation, use of glycemic index, and management of hyper- and hypo-glycemia. Course content for both newly diagnosed diabetics and those wanting better glycemic control. Pharmacists conduct this program with a dietician consultant. Hours are VERY flexible, and classes are conducted with only one patient in attendance.
Members may be referred by physician, or they can call directly and the pharmacist will call physician for a referral. No co-pay for covered members.
Regional Diabetes Center
Comprehensive, standardized program which covers diabetes basics including carbohydrate counting, meal planning, symptom management, blood glucose testing, and dietary management. Dieticians conduct this program. Some refresher courses offered for patients who have undergone recent change like the addition of insulin or an insulin pump.
Members need to be referred by their physician. No co-pay for covered members.
Arnett HealthPlans
The Health Plan provides two programs:
» The first includes a series of workshops with various topics including skin care, relaxation techniques, getting the most from you annual MD visits, home management, exercise, diet, recipe adjustments, label reading and substitution. Other topics include understanding insulin resistance, nutrients and fiber, and medication management.
» The second program includes individualized counseling and follow-up with the Health Educator or through the Personal Care Program. Members receive education about problem areas. They are empowered to make better choices about their diabetes management. A Registered Nurse conducts these programs.
No referral required. No co-pay for covered members.
* Also covered by HMO is consultation with a dietician at Arnett Clinic, Home Hospital, St. Elizabeth Hospital, Jasper County Hospital, and Clinton County Hospital.
ASTHMA EDUCATION
Arnett HealthPlans
This program includes small group teaching, individualized counseling and follow-up with the Health Educator. Members receive education about problem areas in symptom management. They are empowered to make better choices about their asthma management. A Registered Nurse conducts these programs.
No referral required. No co-pay for covered members.
Lincare
This program includes individualized counseling and teaching by a respiratory therapist. There is also a home assessment performed to identify environmental triggers. The teaching consists of 1) developing an action plan with the patient and physician (RED-YELLOW-GREEN ZONES), 2) use of medications and all inhalers, and 3) symptom management.
Physician referral is necessary. No co-pay for covered members.
CHF EDUCATION
Lincare
This program is provided at no cost to members or the Plan and is offered in the patient's home. Members are provided a scale if they do not have one. A Respiratory Therapist or Registered Nurse does an in home assessment and education regarding CHF. Lincare communicates with the practitioner any concerns or recommendations to the existing treatment plan.
COPD EDUCATION
Lincare
This program is a new program that is provided at no cost to members or the Plan and is offered in the patient's home. A Respiratory Therapist or Riegistered Nurse performs an in home assessment, included is an overnight pulse oximetry. Education regarding COPD is also provided. Lincare communicates with the practitioner any concerns or recommendations to the existing treatment plan.
CHOOSE TO LIVE WELL
Arnett HealthPlans
A unique self-management program designed for people with chronic conditions. Chronic conditions may include breathing problems, heart disease, stroke, cancer, diabetes, arthritis or high blood pressure. This six-session workshop helps people cope with the symptoms and frustrations of living with chronic conditions. Topics covered: working with your health care provider, problem solving, the benefits of fitness/exercise, relaxation techniques and managing negative emotions and symptoms. All classes are lead by two trained members of the community. At least one of the leaders is living with a chronic condition. The classes are conveniently located in the community and are at no cost to participants. You do not need to be a member of Arnett HealthPlans. |
