In order to help you manage your continuing care, Arnett HealthPlans is pleased to introduce the Personal Care Program. The main goal of the Personal Care Program is to help members with unique, complicated, or chronic medical needs lead a healthy, active life. Members are identified by Arnett HealthPlans or may be referred to the program by their Primary Care Physician (PCP). Arnett HealthPlans has Personal Care Coordinators who are registered nurses with extensive medical backgrounds and are knowledgeable in community resources. Members participating in the Personal Care Program will receive an introductory phone call from the Personal Care Coordinator. A plan of care specific to each participating member will be developed and coordinated with the member and his or her PCP.
