Choose to Live Well
Arnett HealthPlans would like to introduce an exciting new program that is open to anyone living with a chronic illness. You not need to be a member of Arnett HealthPlans to participate.
"Choose To Live Well" was designed by Stanford University and provides a unique self-management approach to educating people with chronic conditions. Chronic conditions may include breathing problems, heart disease, stroke, cancer, diabetes, arthritis, or high blood pressure. Research studies have shown that the course is extremely effective in helping people manage their health related problems.
The workshop consists of six sessions that meet weekly for two-and-a-half hours each. Class size is limited to 15 participants, and the classes are offered at no cost to the patients. Active involvement is encouraged in topics including problem solving, the benefits of exercise, managing negative emotions and relaxation techniques. Two trained leaders from the community help direct the workshop. At least one of the trained leaders lives with a chronic condition. The workshops will be conveniently located within the community and support people such as a spouse, friend or family member are encouraged to attend with the patients.
To participate, you must have at least one chronic disease, have no hearing or cognitive deficits and be willing to live a better life.
