These are valuable educational services, available to you at no cost. Our Health Education staff works with members who have chronic diseases, especially asthma. This additional education enables members to make informed choices and have more control over their health-care. Disease Management activities provide up-to-date medication and treatment information, as well as ideas for dealing with a chronic condition on a daily basis. Disease Management also provides a checks and balances system, reminding members about annual recommended services. Our activities do not replace regular visits with your primary care physician.
