Diabetes Management

Monthly Classes cover a range of topics from menu planning to stress reduction. Participants have time to ask specific questions and talk with others about challenges. Members can attend either morning or evening sessions. The Disease Management Nurse also provides disease-specific information so members have more control of their lives!

In 2002, classes in Crawfordsville, Rensselaer, Monticello, and Frankfort will be available for members.

Getting Your Groove Back (weight management and annual exams)

Dining Out (Finding good choices)

Taking Time Out (stress reduction and exercise)

The Skin You're In (skin and foot care)

Trick or Treat (label reading and glycemic index)

Family Re-Unions (making meals diabetes friendly)

All diabetics receive quarterly newsletters that contain information related to diabetes care. In addition, we will remind you of the basic services that you should be receiving and encourage you to get these services as recommended.

Any diabetic with a Hemoglobin A1C above 9.0 or other health problems is considered to be at high risk for diabetes complications. This special group receives additional newsletters, which provide intensive information about improving diabetes management.

Recommended Services for Diabetes:

Hemoglobin A1C Blood Test (annual or more often if above 9 mg/dl)

Total Cholesterol Blood Test (including LDL - annual)

Micro-Albumin Urine Screening (annual unless person has known kidney disease)

Dilated Retinal Eye Exam (annual)

Foot Exam (at every doctor's visit for diabetes)

If you have diabetes and have not received any information from the Disease Management Program within 3 months of enrollment, please contact our Member Services Department.
